Description:
Manage nursing care for an assigned caseload, support field nurses, and ensure timely, accurate, and compliant documentation. Collaborate with patients, families, physicians, insurance case managers, and our interdisciplinary team to facilitate appropriate, high-quality care.
Job Duties:
- Oversee and coordinate nursing care for assigned patients.
- Guide and support field nurses; serve as a clinical resource.
- Review clinical documentation for accuracy, completeness, and compliance.
- Communicate with patients/families, physicians, insurance case managers, and internal teams to advance safe, timely care.
- Participate in quality improvement and adhere to agency policies/standards.
Requirements:
- Qualifications.
- Valid NY RN license in good standing.
- Associate’s degree required; BSN preferred.
- 2+ years nursing experience (minimum).
- 1+ year home care experience (required).
- Strong communication and computer skills.
Compensation & Benefits:
- Salary: $85,000 (annual).
- Medical: Individual medical coverage with a minor employee contribution.
- Paid Time Off: 4 weeks PTO annually.
Schedule: Full-time, 5 days/week, 35-40 hours per week
